If you or your child are struggling with attention, focus, impulsivity, or organization, an ADHD evaluation can provide clarity and direction. I offer structured ADHD assessments for both children and adults in the South Bend area using well-researched clinical tools and a supportive, straightforward process.

What is an ADHD Evaluation?

An ADHD evaluation is a structured process that looks at attention, executive functioning, behavioral patterns, and life history to determine whether ADHD is present and what supports may be helpful. Evaluations are individualized and designed to be thorough without being overwhelming.

Cost

The total cost of an ADHD evaluation is $375.
This includes all appointments, scoring of assessment tools, and a written summary of findings.

Tools & Methods

Evaluations may include:

  • DIVA-5 (3rd Edition) – Structured diagnostic interview for ADHD in adults and youth

  • CAARS-2 – Standardized ADHD rating scales for adults

  • Conners 4th Edition – Standardized ADHD rating scales for children and adolescents

  • Clinical interview and background history

  • Review of academic, occupational, or behavioral concerns as appropriate

These tools are widely recognized and help ensure an accurate and balanced assessment.

The Process

Most ADHD evaluations are completed in 2–3 appointments and include:

Visit 1 – Intake & Background

  • Review of history, current concerns, and goals

  • Discussion of symptoms and daily functioning

Visit 2 – Structured Assessment

  • Completion of interviews and rating scales

  • Additional information gathering if needed

Visit 3 – Feedback & Report

  • Clear explanation of results

  • Written summary and recommendations

  • Opportunity for questions and next-step planning

Some evaluations may be completed in two visits depending on scheduling and complexity.

Fees & Payment

ADHD evaluations are a private-pay service. Payment is due at the time of each appointment as the evaluation progresses. Lump-sum payments are not required.

While I cannot guarantee insurance reimbursement, I will provide clear documentation and support so you have the best opportunity to pursue any available benefits through your insurance provider.

Insurance & Reimbursement

I am out-of-network with all insurance companies. While I do not bill insurance directly, I can provide a superbill that you may submit to your insurance provider for possible reimbursement.

Please note that reimbursement is not guaranteed, as coverage varies widely by plan. I will provide commonly used billing codes in advance so you can contact your insurance company and verify your benefits before beginning the evaluation.

Flexibility in the Evaluation Process

The evaluation process is collaborative and may be paused or discontinued at any point if appropriate. If significant concerns arise that are outside the scope of this service, you will be informed promptly and referrals to other providers or treatment options may be recommended.

If the evaluation is paused, previously quoted pricing will be honored for up to 3 months following your most recent appointment, allowing you the option to resume without unexpected cost changes.
